Summary
This World War I diagnosis tag booklet was designed for physicians of the U.S. Army for use among sick and wounded on the battlefield. It provides a carbon copy record of each wounded or sick soldier examined. The cover is marked "Edition of 1917" and the first seven carbon entires were recorded by Dr. Duggins. Several are marked on the reverse with Base Hospital #6. Diagnoses include Influenza, acute epidemic; chancroid; bronchitis, acute?; and undetermined. Dr. Percy Elisha Duggins (1890-1965) was a 1913 graduate of the UVA Sschool of Medicine.
